If you do not provide the LA notification letter with the form below, a copy of the letter must be sent via email to register and process your request.

Please note- In-line with SEND First-tier Tribunal hearings through Her Majesty’s Courts and Tribunals Service, all mediations for the foreseeable future will be ‘remote mediations’ via our Connect Mediation platform.

* Please note, if the young person is over compulsory school age, we will also require a signed letter of consent explaining they are happy for you to manage the appeal process on their behalf. An example of the consent letter can be found here
